What is a Pediatrician?
A pediatrician is a healthcare provider whose specific specialty is caring for children from birth to 20 years of age.(we accept new patients up to the age of 14 years) Pediatricians address acute and chronic illnesses, learning disabilities, behavioral issues and the growth and development of children. Our goal is to help ensure that children grow into mentally and physical healthy, productive and happy adults.

Third party requested forms
All patients must be up to date on vaccinations and preventative exams (exams must be done annually, if patient under the age of the three years then according to Bright Futures schedule) for our providers/staff to complete forms. As a courtesy at your child's annual physical exam, Pedicorp providers you with a school form. It is your responsibility to maintain a copy for your personal records. Should you require another form we will charge you a form fee, this fee is per form. Our office provides the service of thoroughly reviewing each medical chart along with each form. Our required signature indicates we have completed and reviewed the information filled out on the form. This for the health and safety of our patients.
According to the Connecticut Department of Public Health patient-requested forms such as blue forms/yellow forms/camp forms are separate from the medical records and that a healthcare provider can charge for patient-requested forms.
All requested forms may take one to two week(s), time of year dependent, to complete.
The completion of forms is not a billable service to insurance companies, including Medicaid.
